Our Mission:  To provide compassionate, caring service, taking into consideration the needs of all who are affected by the later stage of life.

Services for Families and Elders

  • In-home evaluation of the elder(s)
  • Consultation/counseling with families or significant others
  • Individualized care planning that takes into consideration the physical, cognitive, emotional, social and spiritual needs of the elder(s)
  • Follow up phone calls to ensure that the plan of care is successfully implemented
  • Conflict resolution/counseling for family members who are experiencing a conflict or impasse about care decisions of an elder 
  • Advocacy services provides an in-person or telephone "advocate" on behalf of families or elders when they encounter challenges or conflicts with a facility
  • Case managers provide on-going assistance with services for the elder, including check-in visits, medication monitoring, escort to appointments,  and other services as needed or desired
  • Phone consultation, conference calls and email correspondence for families who live out of town
  • Referrals to elder care facilities, home health agencies, hospices, Geriatricians, elder-law attorneys and other Geriatric professionals and services

 

Services for Professionals
  • On-site consultation and training
  • Needs based training tailored to each facility
  • Coaching on how to effectively motivate staff and encourage personal and professional growth 
  • Consultation about difficult staff situations, such as, supervision, staff relationships, staff/family conflict

  • Follow-up with the client(s) to eldercare facilities to monitor consistency of care
  • Ethical practice as governed by the National Association for Social Workers, the National Association of Geriatric Care Managers and the National Association for Marriage and Family Therapists
  • GCS does not accept referral fees from assisted living or other organizations
